These partnerships allow us to connect clients and families with additional services that complement Landis at Home care. Whether someone needs help making their home safer, support after an illness or injury, or guidance through a transition, our partners help create a more complete network of care.

Brubaker Partnership

Supporting safer, more comfortable living at home

Landis at Home has partnered with Brubaker Inc. to support older adults who want to age gracefully and safely in their own homes.

Through Landis at Home’s Graceful Transitions program, an occupational therapist who specializes in aging in place can provide home safety assessments, personalized recommendations, and support with home organization, packing, decluttering, and redecorating.

Services may include:

Home safety assessments and aging-in-place recommendations
Repairs, renovations, and home adaptations
Plumbing, electrical, and HVAC support
Kitchen, laundry, and bath remodeling
Home organization, decluttering, packing, and setup support through Graceful Transitions

Comprehensive Range of Services

When repairs, renovations, or home adaptations are recommended, Brubaker serves as a preferred provider to help complete that work. Their team brings expertise in plumbing, electrical, HVAC, and aging-in-place certified kitchen, laundry, and bath remodeling.

Together, Landis at Home and Brubaker help create home environments that are safer, more comfortable, and better suited to changing needs.

Symbria Partnership

Bringing rehabilitation support into the home

Landis at Home has also partnered with Symbria, a rehabilitation provider, to expand access to therapy and support services for clients living beyond the Landis Homes campus.

Symbria’s outpatient therapy at home program brings rehabilitation services directly into a person’s own home environment. This helps clients receive support in the place where they feel most comfortable, while also allowing therapy to be connected to their everyday routines and goals.

Services may include physical therapy, occupational therapy, speech therapy, caregiver support and education, and coordination among the key people involved in a client’s care.

This partnership also strengthens communication between Symbria providers and the Landis at Home team, including nurses and support staff, especially when a client experiences a change in needs.
