Whether you have received a new diagnosis, are managing a chronic condition or need extra support coordinating care, Landis at Home can help. Through Nurse Clinical Support, a Landis at Home nurse can become a “go-to” person for clients and families, helping oversee care, support communication and provide scheduled clinical services.

These services are not intended for emergency care. Visits are scheduled ahead of time and are available to Landis at Home clients.

Nurse Case Management provides ongoing support for clients who would benefit from regular check-ins, care coordination and help managing health needs at home.

Nurse Case Management may include:

New diagnosis support, disease management and monitoring
Nurse assessment of the client
Communication with family, physicians and service providers
Coordination with home health, pharmacy and other care partners
Support during the transition home from a hospital or rehab stay
Assistance with scheduling appointments
Coordination of care and services
Help identifying needs before they become a crisis
Chronic disease management and monitoring, such as diabetes, heart failure or COPD

Clinical Support

Clinical support is available through Landis at Home nurses and may be provided one time, on an ongoing basis or as needed. Skilled visits are scheduled ahead of time and can help clients receive practical nursing support in the comfort of home.

Clinical Support may include:

Ear lavage
Injections and medication administration
Intermittent straight catheterizations
Wound care and dressing changes
Tuberculin skin testing
Assessment of a medical condition
Ordering long-term-use medical equipment and supplies, such as diabetic supplies, wound dressings or catheterization kits

Clinical assessments are not to be used for emergency situations or in place of a primary care provider or emergency department.

To utilize Nurse Clinical Support, you must be a Landis at Home client. To become a client, one of our nurses will complete paperwork with you, assess your needs and create a customized plan for your care.

Nurse Case Management and Clinical Support are available during regular business hours, Monday through Friday, 8:00 AM to 4:30 PM. Services are provided in 15-minute increments. Services needed outside of regular business hours may be available based on nurse availability.
